When travelling abroad, does a child need written parental consent if travelling unaccompanied?
If the country in question requires such confirmation, it is listed on the website of the Ministry of Foreign Affairs of the Czech Republic, in the chapter Information on conditions of travel to individual countries of the world, in the section dedicated to a specific country. In order to verify or possibly supplement this information, it is necessary to contact directly the embassy of the country in question in the Czech Republic. Even if this document is not required by the other state, the Ministry of Foreign Affairs of the Czech Republic recommends that whenever a minor citizen of the Czech Republic travels abroad without the accompaniment of his/her parent or legal guardian, the accompanying person should have the written consent of the parent for the specific trip.
In the written consent, it is advisable to identify the child sufficiently (name and surname, date of birth and travel document number), as well as the accompanying person, the destination and the approximate date of travel. The person giving the written consent should have his/her signature notarised or verified by a notary public or municipal authority. For very young children, a photocopy of the birth certificate may be attached. In addition to English, it is recommended to give written consent in a foreign language applicable in the destination for which it is intended (download the form below).
